Ultrasound guided vascular access involves using high-frequency ultrasound to visualize vessels before inserting a needle or catheter. This technique enhances accuracy, especially in difficult vessels, reducing the risk of complications associated with traditional venipuncture methods using landmark or palpation techniques.
Benefits of Ultrasound Guidance for Vascular Access:
1. Increased Success Rates: Visualizing the vein in real-time helps in precise cannulation, minimizing the need for multiple needle sticks.
2. Reduced Complications: It lowers the chances of damaging surrounding structures and reduces the incidence of hematomas.

Key Techniques for Ultrasound-Guided Vascular Access:
1. Choosing the Right Equipment:
Investing in quality equipment like a good ultrasound system can significantly impact your success. Look for features that offer high-resolution imaging and user-friendly interfaces.
2. Preparing the Patient:
Ensure the patient is properly positioned and the skin is prepared with antiseptic solutions. Proper positioning enhances vein visibility and accessibility. Clinician, cannula, vein and ultrasound machine should be in a straight line. This can be achieved by placing the ultrasound machine on the opposite side of the clinician who is performing the cannulation.
3. Locating the Vein:
Use the ultrasound probe to locate the vein. Adjust the probe’s angle and depth settings to get the clearest view. Remember, ultrasound can be an invaluable tool in this process.
4. Inserting the Needle/ cannula:
With the vessel clearly visible on the screen, insert the cannula using a technique that minimizes discomfort and maximizes accuracy. The cannulation procedure should be steady and precise. Application of EMLA or local anesthetics prior to the procedure helps the most in such cases.
5. Confirming Placement:
Once the catheter is in place, confirm its position by checking for blood return and re-evaluating with the ultrasound.
Advanced Techniques and Tips:
- For vessels that are hard to locate, consider using a high-frequency probe and applying gentle pressure to increase vessel prominence.
- Stay calm and steady.
- Practice makes perfect, so continually refine your technique through training and practice.

      Great question! Thank you! The time taken per paper depends on multiple factors: 1) type of paper, 2) quality of paper, 3) your reading speed, 4) complexity of data , 5) the type of article you are writing to name a few. My approach is to read the abstract thoroughly at first to get a helicopter view of the article, then go through the methods, results and discussion. Finally go through the entire article. If you follow this strategy, it may take 2 to 3 hours per paper. There is also a short cut version of each steps. Perhaps, I will do another video on that topic. Cheers! Dr Barai

      Great question! The best thing to do is to write the critical appraisal as per the tool kits. The way it reads better is by saying 1) what the authors have written, 2) what you think including the strengths and weaknesses followed by 3) an analysis as to how it is going to change your management if at all and finally 4) some recommendations.     I want to ask that the pixel/inch details are not showing up for me, what is the reason?
